It's not linked to a question bank. I put new responses in for each option. I also just cleared my cache, which didn't help. I still get between 4-10 empty choices added to every question. 

I keep getting empty choices for the MC options. I go back to edit, hit the little garbage can to delete the empty ones, update the question, and save. When I go back to review, the blank spaces, and ...
