I've spent so many hours of my life thinking about question banks and outcomes! I'm still using the older quiz type, New quizzes is a whole other mess.
From my experiments, the mastery gradebook treats the items pulled from each question bank as a separate entry into the mastery gradebook, and calculates mastery based on just those items. For example, in this test I had three question banks, all aligned to the same outcome, and pulled one question from each bank. The mastery gradebook treated each bank separately, scoring either a 4 or a 0. The decaying average calculation system means that this student got a 3.3, but a student who got the third question incorrect would score a 1.2. Not at all what I want. I'm now experimenting with adding rubrics to each quiz so that when I grade them, I can also select a mastery level. It's not all nice and automatic, but it's the only way that I've found to have an entire quiz considered as one outcome.

Yes please! I'd like a course level setting that lets the instructor choose where the new modules go, as well as a notice on the modules page itself to let students know which setting the instructor picked!

I'm pretty sure that it changed when the rich text editor launched. I used to do keyboard link entry in the old text editor all the time while using chrome.

I would like to be able to use keyboard shortcuts to post a new external link in the new rich text editor. Currently I can select text and use Ctrl+K to bring up the add link popup, but I need to click the field to paste in the link.
Could the add link popup default be changed so that the cursor is in the "Text" box when no text is selected and in the "Link" box when text has been autofilled?
Adding these default cursor positions would allow users to create external links without having to use the mouse.

You could just adjust the number of points per question so that the total come out to what you want. 5 points spread out over seven questions is 0.72pts per question. When you grade, you give 1 point for each of the selected questions, and zero for the two non-selected questions.
