We have just had about ⅔ of our profile pictures disappear as well.  Anyone have any idea what is happening?

I am randomly getting the same thing in several of our courses. I have faculty contacting me about it, but they are not using LTI tools in their course. 

Is there a way to have apps disabled by default in a course?  We've added a tutoring app that faculty can use for their course if they want. But as of now, the app is in the "Enabled" area...
