On the Modules page, when I add an existing item such as a discussion that I already created, it only appears once on the Discussions page. I want to be able to have the same discussion prompt each week and grade them separately.

Then you will need to create an individual graded Discussion for each week. If you are using the same basic discussion prompt each week, then you could simply copy (duplicate) the original Discussion then change the title slightly to match the week - like, Week One Discussion, Week Two Discussion, etc, or whatever naming conventions work for you and your students.
