I have not seen the ability to globally rename a file anywhere once it is in use in a course. When this has happened to me, I use the drop down list of locations under the video to edit each one individually.
Because of that frustration, I've altered my workflow, but now it works and I don't have the problem anymore. I always upload to My Library and never directly into a course. I've got too many courses named the same thing (within the allowed characters in the course list of Studio) and so I put everything in My Library and use collections or tags to organize. After I upload the video, I make sure that I edit the video and set the title, description, and tags before I embed it in a class.
I think what we're encountering is flexibility by design. A video can be used in multiple locations and people may want to call it different things in different courses as the purpose of the video changes. I may want to tag it differently depending on the course (it might be in chapter 2 in one course but chapter 15 in a different course) as well.
I see a similar design with the renaming of a content page title within Canvas that does not automatically update all of the links. Many (probably most) people would want that functionality and many (probably most) would want that here. There are cases where you don't want that functionality, though, and if it's forced then there's no way to satisfy those cases that didn't want it. One of the places Canvas does that is when publishing a module automatically publishes the items under it -- that's forced, even if we don't want it.
In a perfect world, everything would be carefully planned out and you would have a naming scheme in place that considered every possible fringe case before you ever create a single piece of content. I certainly don't live there and I'm not sure I would even like to visit.
