I've run into a strange issue with New Quizzes. We have content Outcomes available at the root level. Teachers are able to import those as needed into their courses. I'm working with a staff member now who has all course outcomes imported and available, except in New Quizzes.
In the New Quizzes picker, all the folders are there and B.3 has three of the five Outcomes showing. We cannot figure out why the rest are blank. This is on a fresh quiz (not imported) and the items are not in banks. The Outcomes have been available in the course since August, so it isn't related to that 24-hour wait period issue from a year or two ago.
Any insights on what to check next?

First, I want to clarify. The 5 outcomes in question are course outcomes, correct?
Next, was the New Quiz in question copied from another course?
If both of these are yes, then you could try duplicating the New Quiz and seeing if that will populate the Outcomes that are missing. You would do so by following this guide, but instead of copying into another course, you would copy the quiz into the same course.
We are trying this out because we have seen some reports of New Quizzes not populating questions after being copied, so we would like to know if this is related.
