Normally this occurs because you have linked to an image that resides in a place you have permission to view, but your students don't. For example, the image might reside in the course files of a previous course, or in your My Files space. You have permission to see that image, so it looks fine to you, but your students don't have permission to view the previous course or your personal files, so they will see the lock.

The quick fix is to upload the file into the same course that contains the quiz, and edit the quiz question to repair the link so that the image now links to the image in the course files.
