[Accounts] Show extra fields in Add Login screen

When editing an existing user login via the Update Login screen, the Login, SIS ID and Integration ID fields are shown along with the Password and Confirm Password fields. However when adding a new login via the Add Login screen, only the Login, Password and Confirm Password fields are shown. Staff who add extra logins and want to add in SIS ID and/or Integration ID fields as part of adding the new login are obliged to save the new login ID and password, refresh the browser tab then reedit the new login. Most of our staff are thus unaware of the SIS ID and Integration ID fields and so many logins are manually created without these fields. This then requires our admin staff to continually retrospectively add these in in order to perform bulk operations on user accounts, of which at least some depend on identifying user accounts via user SIS ID field rather than the Login ID field
Change the Add Login screen to include SIS ID and Integration ID fields along with the existing Login ID, Password and Confirm Password fields. Add an option to make entering values for each of these fields compulsory or not in order to be able to save the new login
