[External Tools] Improved LTI Customization for Course Navigations

Problem statement:

When adding an LTI you can choose its Placements. One of those options is adding it to all Course Navigation menus. This creates an issue when the LTI is not something all courses would benefit from like a publisher LTI that allows an instructor to connect their publisher course with their Canvas course. Not all instructors will use this LTI and its addition to all Course Navigation can cause student confusion regarding this menu addition and what they must do with it. To avoid navigation disruption, admin may choose to avoid using the course navigation placement for one tool. This creates a usage inconsistency for both students and faculty. For example, if a new publisher is added to Canvas and is not added to the Course Navigation to avoid navigation disruption, it's usage will be inconsistent with the usage of previous publishers that were added during implementation. Students who are used to using the Course Navigation to connect with publishers will need to look elsewhere for this connection information, and instructors need to learn another way to add publisher to their course.

Proposed solution:

When adding the placement of an LTI to Course Navigation have an additional option of whether the LTI is enabled or disabled on all Course Navigation Menus by default. This allows the instructor to turn on the LTI if it is relevant to their course by going to Settings in their Course Navigation menu and then clicking Navigation. This also allows for consistent placement without disrupting all Course Navigation menus.
