[New Quizzes] Notation on Question in Item Bank that it exists in the Test

Problem statement:

When teachers add a question from an Item Bank into a test, there is nothing that indicates the item is already added to the test. This has caused teachers to unknowingly add the same question multiple times to a Quiz (New Quizzes).

Proposed solution:

Add a descriptor next to the question title in an item bank to notate that it already exists in the test, this will prevent teachers from accidentally adding the same question multiple times.
