[Speedgrader] Enhance comment bank

Problem statement:

The current comment library is limited in its use as it can only be using within the comment box and even there with limitations. It can't be used to add feedback on the paper or in the rubric. To save time tutors have to store feedback on word documents and use copy/paste. This is inefficient and time consuming.

Proposed solution:

The comment library should be expanded to allow 'folders' of comments, with 'folders' created and named by the marker. For example, a set of comments for Assignment 1, and another set of comments for Assignment 2. Comments in the library should be usable within the comment box, as now, but it should be possible to leave multiple comments and not have them overwrite existing content. Comments in the library should also be usable for leaving feedback in the document viewer, on the submission.
