I have not seen this before. A glitch is possible, but more likely to be on the instructor's end. A few things to try: reload the browser, try a different browser, reboot the computer and try a different computer. Basically, we want to try and eliminate possible reasons that are on the instructor's end before examining if it is on Canvas's end. Particularly since this instructor may be the only one experiencing this issue. If others were experiencing it, or if she starts seeing it on other devices, then I might be more likely to suggest submitting a help ticket to Canvas. If you do submit a ticket, make sure to list what was tried before submitting.
