When I bring up an assignment on Speed Grader I do not get a menu bar to allow me to annotate the document submitted by the student. Why? What setting do I need to change?

It could be that the submitted file is not compatible with the annotation tools. See this document How do I add annotated comments in student submissions using DocViewer in SpeedGrader?
It does note that annotation tools are not available for text entry assignments.
If it is one of the supported file types, then I would recommend opening a ticket with Support from Help > Report a Problem.
