anonymous midterm student course evaluation

Is there a way, on Canvas, for me to administer an anonymous midterm student course evaluation?  I'd like to get some feedback about my teaching from the students in the form of a multiple choice evaluation with a few open text fields.  I'd like to allow students to submit their thoughts anonymously.

Hello Warren,

You could use an anonymous survey:https://community.canvaslms.com/docs/DOC-13126-415268346 

However, you would still be able to see who has or has not filled out the survey.
