This behavior has been resolved and deployed to the production environment as of 11/05/25.
When linking to a course file from the RCE in a Question Bank question, after saving, the file gets a verifier added which prevents the file from previewing when selecting the file.
The download option next to the file link still works to download the file.
The file us unable to be previewed both in the question bank and in quizzes where the question is linked.
Instead, files added to question banks should be able to preview successfully.
Edit the question again and remove the verifier manually.
Prerequisite: A course and file.
Upload a file to the course.
Create a Question Bank and select the file to be added in the RCE as you create a question.
Save the question and notice the file link shows an error and is not accessible.
