What might allow students to work past their set time limit in New Quizzes Moderation?

I have a student who was given additional time on an exam in New Quizzes -- time their instructors did not set for them. While the rest of their classmates who received the same 'time multiplied by 1.5' extension were given one hour to take the exam, and this is the time reflected in the Assessment Settings section of their Moderate page, the time column shows that they took eight additional minutes. (See photo below in yellow highlighted areas; names and scores removed for privacy.)

While she only went over by 8 minutes, the instructors tell me the total additional time she was given was 15 minutes, and they don't know where it came from. 

Does anyone know how this might have happened, and what can be done to prevent it? Thanks in advance!

I am not sure I followed your question completely as far as how the student was moderated.

However, from the information you sent in the picture, there is nothing wrong with what New Quizzes did for the student.  The student highlighted was moderated to receive 1.5X the original time on the quiz.  What you highlighted on the right shows that the regular assignment has a time limit of 1 hour.  Therefore the moderated student will receive 1.5 hours.  so everything reported about the time allowed the student is correct and nothing is wrong with what Canvas did.

The one part that confused me was the statement that the student received the moderated time, but the teacher did not give it to them.  If that is the case, then my only assumption is that the teacher selected the wrong student - I have never seen a student receive moderated times automatically.

As for something else that I think I saw in your question.  If a student has moderated time of 1.5X and the regular time is 1 hour (student is to receive 1.5 hours), then the student must start the quiz 1.5 hours before the until time.  if the student starts the quiz with less than 1.5 hours before the until time, then they will not receive the full 1.5 hours - they get however much time is left to the until time.
