I am looking for a way to convert an old hardcopy paper assignment to be on canvas and accessible, and am kind of stumped.

The idea is that students have completed a small research project, and then afterwards need to reflect on the quality of their sources. So i have a chart in which each column is designated for one of their sources, and each row is a different criteria on which they should rate their source. And then in each box (source x criteria) there is a likert type response. Hardcopy they would just circle one of the scores within each box. But electronically it gets challenging. 

• Downloading the file is hard because it can be tricky to be compatible and editable.
• Should i put the blank chart in an assignment and ask them to cut and paste onto a document of their own?
• Is there a file format that is best for downloading and being editable?
• Is there a way i can keep the whole thing internal on canvas?

Any ideas?
