Is there a difference between content selector for images and image insertion in the rich text editor?

When working in the Rich Text Editor, I see 2 methods of adding an image, the content selector and the image insertion icon.

Using the content selector, images tab, I get something that looks like this in the HTML view:

When using the image insertion icon in the menu, I get this instead:

<img src="/courses/39014/files/3796091/preview" alt="badger1.PNG" width="339" height="313" />

 

Is there a difference between the two?

An instructor I work with had a problem with images not displaying (which I thought had been fixed in the 02-03-2018 release). I'm wondering if it could have anything to do with how the image was added to the quiz. He was using the image insertion icon.
