What you are describing is new to me, I definitely have not experienced that. My first thought is that it might be a browser caching issue. I would recommend you and whichever students are experiencing clear their browser cache for all time and then restart their browser.
Does it always happen with the same module content? If you add new content does it happen with that too? You might try removing the content that is affected and then adding it back to the modules. Removing it from the module will not remove it from the course, it will only remove the shortcut. I would try that is clearing your browser cache doesn't help.
