How can I create an assignment with questions that students can answer directly so when I grade the assignment I see both the question and their response to the question, i.e.

question 1.  What is your name?

Student response:  Joe Miley

2.  How old are you?

21

All information appears in one contained space.

Hi @joemiley 

I would create that assignment as a quiz using Fill-in-blank question items. The quiz can be graded or ungraded.

New Quizzes:

• How do I create an assessment using New Quizzes? 
• How do I create a Fill in the Blank question in New Quizzes?
