[Courses] Additional fields on Canvas Provisioning report

Problem statement:

Currently the courses provisioning report has these fields: canvas_course_id,course_id,integration_id,short_name,long_name,canvas_account_id,account_id,canvas_term_id,term_id,status,start_date,end_date,course_format,blueprint_course_id,created_by_sis

Proposed solution:

Proposal is to include a blueprint_canvas_course_id before the blueprint_course_id. This makes the identification of blueprint courses easier.

I think this is an excellent idea and would make the format more consistant.

Problem statement:

Cant find out when a course was created, or when a user was added into Canvas

Proposed solution:

Be able to export a provisioning report about users and courses, where it states dates. Like when created / added to Canvas.
