[Gradebook] Notate state of Manual posting in Gradebook

Problem statement:

When an instructor has the grade posting policy set to Manual, and then posts grades, they have the option to post grades for "Everyone" or "Graded." By default, "Everyone" is selected. If the instructor remembers to click "Graded", then the manual posting policy remains in place, and new grades are hidden until the instructor manually posts them. This is as it should be. However, the problem is that if the instructor forgets to click "Graded," then grades are posted for "Everyone," and the result is the same as if the instructor set the grade posting policy to "Automatic" instead of "Manual": All new grades from that point forward will be displayed to students. To complicate matters, the MANUAL notification on the assignment column title in the gradebook remains, and there is no visual indication to the instructor that all grades will now be posted automatically. (There is also no simple way to see whether you selected "Everyone" or "Graded." The only way to find out is to enter a grade for a student and see if it is hidden or automatically posted.)

Proposed solution:

I propose one of two solutions: Ideally, I propose that the "Everyone" option be removed, so that when instructors post grades, they are only posted for graded assignments. Reason: the "Everyone" option seems unnecessary, as instructors could simply set the grade posting policy to "Automatic" if they no longer wish to be able to review grades before they are visible to students. Barring that, it would be acceptable if there were some visual notification that all new grades will now be posted for everyone. Removing the MANUAL label from the assignment column title would be sufficient. That way, instructors would at least be able to realize their mistake and fix it.
