[Modules] Deleted Assignment and Discussion Submissions Should Not Count Toward Completion

Problem statement:

A student can currently submit anything for a discussion or assignment, delete it, and still get credit towards the module completion badge (depending on the settings of the module requirements).

Proposed solution:

If a student deleted a submission that gave them credit towards module completion, that module requirement checkmark should be revoked until the student resubmits and fulfills the module requirement.
