[New Quizzes] Printing Quiz name prompt

When printing a quiz it would be great if it automatically included a prompt near the top for the student to write their name. Something like “Name:_____________________”

To expand this a bit, it would be great if you could check an option to only print the questions they got wrong when printing results of a quiz instead of printing everything.

Problem statement:

When printing a student's submission details report, the student's name, course name, and quiz name are included on the screen, but aren't included once you click Print Report. Assuming a hardcopy of this report is necessary for some kind of record, it's not very useful without the aforementioned details.

Proposed solution:

We have a new Aviation Maintenance program, and the FAA requires that the instructor keep hardcopies of student quiz results. These are rendered useless without the student name, course name, and quiz name included in the report. Since this information does appear on the screen, if the programmer would include that information when the Print Results button is clicked, it would be appreciated and very helpful. Thank you, Beth
