[New Quizzes] We need a printout of a student's graded quiz that includes the student's name

Problem statement:

We have a new Aviation Maintenance program. The FAA requires that our aviation instructor keep hard copies of all students' graded quizzes. The instructor chose to use New Quizzes so he would have the PRINT button. Currently, when a quiz is printed using the PRINT button, the printout doesn't include the student's name, which makes the printout unacceptable for record purposes. The name does appear on the printout when using the browser print feature (3 dots at top right), but it will only print the first page of the graded quiz, which is also not acceptable for FAA records.

Proposed solution:

Please include the student name field on the printout when using the PRINT button in New Quizzes so our aviation instructor can meet FAA requirements.

Problem statement:

Currently, new quizzes will allow teachers to print a hard copy of their students' assessments; however, this print out does not include the student's name or the name of the assessment itself. At present, teachers who want to print hard copies of their assessments (i.e., work samples, progress folders) have to hand-write this info on the print out.

Proposed solution:

An option needs to be made available (a toggle button, perhaps) to have the student's name and the name of the assessment appear on the print out.
