Align outcomes to individual quiz question issues.

I'm having issues aligning outcomes to individual quiz questions. It appears correct when building the "New Quiz" and I have gone through all the correct processes so that under each question there is an aligned outcome, however when a student takes the quiz and then afterwards needs to check their learning mastery in "Grades" it shows all the outcomes but none of them are aligned.
If I then check the "Outcomes" section, and go to alignments, again it states that none of the outcomes have any alignments.
Has anyone else had a similar issue and or know a possible solution?
