Application to manage accessibly request that Integrate with Canvas?

We are looking for a management system that would help track and manage accommodation requests for students with disabilities.

This system could let the students schedule an appointment with a counselor, would let the counselor keep all relevant data tor each students. It could let the student request accommodation for his exams, so an integration with Canvas would be ideal.

What do you use to manage accommodation for your student and avoir repeat and manual data entry?
