Canvas is not deleting questions from question bank . . . and MORE!

I'm using Classic Quizzes.

Actually, two problems:

(1) Canvas is not deleting selected questions from the Question Bank.  Instead, it greys out the question -- but, does not delete it from the bank.

(2) Modifying and updating a question causes Canvas to duplicate the question.  In other words, the original question is not changed or updated.  Rather, Canvas simply leaves the original question un-updated and creates another distinct, new question which is in fact "udpated."

Okay -- annoying, but fine.  All I'd have to do is delete the original, un-updated question, right?  No.  See (1) above.

Anyone else having either of these problems?

Do you have a quiz that has already used the questions from the question bank that you are trying to edit or delete?

In some cases, yes; in some cases, no.  In some cases, the deleted questions were copies from another quiz bank.  But, in some other cases, they were questions that I had just freshly created in the existing bank, and was attempting to modify.
