Missing Module Assignment but in Gradebook and Assignment Tab

I have an assignment that has diappeared from my module. It is still listed my gradebook and it is still listed in the assignments tab but it is NOT in the module anymore. It was in my module but then just disappeared. But if I try adding it back in to the module, it creates a new assignment.

Are you using this Guide to add the assignment back to your module?

How do I add course content as module items? - Instructure Community - 1157 (look at the section about half-way down called "Add Existing Item")
