Question Bank is not "linked" in Quiz

When creating a Quiz I am linking to several Question Banks that I have created. One of the Question Banks does not show up "linked" in the Quiz and is there for "not counted" - so my point total is 90/100 instead of 100/100. Why would a Question Bank not show as a link in the Quiz?

Did you copy the quiz from one course to another? I've seen it happen with copying quizzes that sometimes linked question banks get "broken". The question group will there saying "pull 10 questions..." but there is not bank linked to it. About the only fix I've found for that is to remove the question group and re-add it, linking to the appropriate bank.
The other possibility I can think of is if you forgot to link it to a bank when you created the question group.
