closed captions on videos not working

I followed the directions for uploading captions to a video and all the captions show up in the very beginning of the video. Does anyone know why this is happening?

Are you formatting your file so that it looks similar to what is shown on this site?

My assumption (from your attached screenshot) is that you put all your text into one section of time rather than separating them out.
