I have a single question graded survey in Classic Quizzes that is set up to award 5 points to the student if they complete it. When I take the quiz as a student, however, the points show up as 5 points out of 1. Why would this be? Is it getting the 1 from the number of questions on the survey? Screenshot below.

I didn't notice before that you can't change the point value for a graded survey question. But I found a workaround.
Edit the survey.
Change the quiz type to "Graded Quiz."
Click on the Questions tab.
Edit the question and change the point value.
Click the "Update Question" button.
Go back to the Details tab.
Change the Quiz Type back to "Graded Survey."
Scroll to the bottom and save.
That worked for me, so hopefully it will work for you also.
I was able to recreate what you're seeing. In my testing, the problem is that the graded survey is worth 5 points, but the question itself is only worth 1 point. This is why it shows 5 points out of 1. To fix it, I edited the question to change the point value to 5 points.
Thanks for your response, clpatter! Unfortunately I am unable to set the point value for the question. It seems to default to 1 point and there is nowhere to change it when I edit the question. I wonder if this depends on the question type - this is an essay question. I guess I just have to live with it and let students know that this is what they'll see when they submit the survey.
