How do you process LTI requests?

Hi,
We are reviewing our LTI/software review process, and I wondered what other Canvas schools do. Currently, we have the faculty member fill out 2 forms. One is a rubric that they have the vendor help them fill out. The other is a department review, which makes sure they have the budget for it.
Many don't like the rubric, so we get a lot of pushback. The rubric asks them to evaluate the software based on several categories, like accessibility, support provided, economic value, etc. There are 3 choices for each category: excellent, acceptable, and unacceptable.
Would anyone be willing to share their process and/or forms? I hope to revise the rubric so we can still gather key pieces of information, but not take a lot of time to complete.
