Incorrect student's work appears in Speedgrader

I received the message below from one of my instructors. I have seen this a few times--the wrong person's work appears in Speedgrader. Is this a known glitch? Is there anything that contributes to or causes this issue? I have removed the student names, but they were adjacent in the Speedgrader list. I am concerned as this could cause incorrect feedback to be sent to a student.

As I was grading student work this evening, I opened [Student A's] grading page, but [Student B's] work came up. Fortunately, [Student B] had put his name on his VIsion Board and his document submission so I caught the name issue. I opened up [Student B's] work then and saw it was the same work as on [Student A's] screen. I went back to [Student A's] page and re-opened it and her work came up this time.

I took a look in the Known Issues and did not see anything related to this listed under grading. It would probably be good to report directly to Canvas support, as it sound like an issue the engineers would need to look into. Please share back anything you learn.

I would agree with what @james_whalley said here, definitely report this to Canvas support if it's a reproducible issue!

Given the info from the teacher that switching to another student then going back to the original one with the issue later came up with the right submission, my guess is that this may have been a browser caching issue on the teacher's device.  It'll be almost impossible to prove this, but if the issue doesn't come back, I think it's a strong possibility and not really an issue with Canvas coding.
