We have a program at my school in which students are able to regain course credit through a teacher created stand alone Canvas courses. We have used the same courses for about 3 years now. New sections are added for each year as we deal with different students in different years. Most of the courses are set up with mastery paths. The problem we are having started this year and has no predictability to it as it doesn't happen in the same course every time or with the same student. The courses have modules that start with a pre assessment that puts students into the mastery path for their work to regain credit. Students report being "stuck" and not able to move forward after completing an assessment. When we check if their name is in the list of assigned to students it is always there.
Over the course of this year we are starting to see this more frequently. Our trouble shooting efforts start with clearing browsing history and making sure their school issued chromebooks are up to date. Sometimes this works. We have also also removed the student from the next assignment and then added them back. Sometimes this fixes it. We have also added 1 fudge point to the assessment and again sometimes this is the fix. The problem is that there is not one fix that works every time. I am the only person in the program with knowledge of how the courses were designed and how to fix most of the issues our students see. We currently have 130 students enrolled in any one of almost 60 courses. The time required to check progress and move students were needed is a large demand and is only made worse when we don't know when a student or even which student may have a problem.

I attended a meeting with some peers on Zoom a few days ago and this was brought up as a discussion item there. My institution doesn't use mastery paths much, so I haven't personally experienced this at all, but I don't think you're alone with this problem. As @dbrace mentioned, reporting this to support, if you're able to, is probably the bets way to get action right now. The more people who report the problem, the more examples support and engineers have to research and hopefully find and fix the root issue.

I have not experienced the difficulties that you have described.
Have you reported this directly to Canvas Support, since they have access to your Canvas environment? If you have, what have they said?
