Outcome-aligned question banks but no indication in Learning mastery

I created a question bank and aligned it with an outcome measure.  I used individual questions from this bank in several different quizzes across the semester. In Learning Mastery Gradebook, there is no indication of mastery; the space in each student's row is blank.  Any idea what I might have done wrong or how to correct it? This was my first attempt at using outcome-aligned quiz questions and I had high hopes for it.

You stated question bank, so likely you're using classic. The order of operations may have affected the outcome. Did you align and create this before students took the test or during/after? Also, after you make one you have to click on "import" in the bottom right (as shown in guide.) Additionally, you can create the outcomes first and then link them :
