Error/Bug When Student Navigates to an Assignment - Only One Student w/ Issue

Hello community! 

I am running into a very bizarre bug with just a single student. All other students in the course are just fine! I see the issue, myself, when I masquerade as the student.

Context:

The student is navigating to a chapter workbook in the course that I have setup as a "Reading assignment + Quiz" using the new quizzes. When she clicks on "Begin" it takes her to an OLD edition of that assignment (with no quiz), but it has the exact same URL as the assignment that is working for other students (with quiz questions). I would have assumed it was a broken link or something, but everything seems to be identical.

• This course is a child course under a Blueprint parent
• I am utilizing Item Banks for these quiz questions
• The student is able to see quiz questions for OTHER assignments just like it for other chapters

Attempts to fix/troubleshoot:

• I've tried un-linking and re-linking the course link in the child course with no changes to her screen
• Gave everyone a 2nd attempt - No changes

Screenshots are what other students see (with questions), and what she sees (no questions & old upload option prior to questions being added).

Has anyone ever experienced this type of single-user bug before? Thank you!

@BrennaBateman It sounds like that student must have opened an attempt before the version was updated. If you have her submit and just give them an additional attempt, she should see the new version. I know you mentioned giving everyone a second attempt, but you didn't mention if she had already submitted the first attempt. If this solution does not work, you will want to open a ticket with Instructure, as that would be really weird.

So odd! What shows up for her under "Moderate"? Does it show an attempt in progress?

Could she have clicked to view it before you changed the assignment to the new questions?

This could some type of persistent session ID (maybe quiz_api_quiz_session_id). I wonder what New Quiz ties the session to.

I think I kind of replicated it, although not in a blueprint-connected course.

1. I made a New Quiz with an item bank and unlimited attempts
2. Took it as a student in Student View
3. Went to the quiz as admin and edited the questions/item bank and added new stimulus questions. It showed me this warning. Clicked Done so my changes were saved.
   
4. Went back to Student View and clicked to take the quiz again for a second attempt. It showed me the original version, not the newer one, as expected. I just clicked to view and did not submit a second time, but it still showed "Resume," for my second attempt, which I bet is what she's seeing.
5. Then I tried to access the new edited quiz by (1) resetting the student view, (2) deleting the Fake Student from the section (under settings), (3) clearing my cookies/cache, and (4) with Act as User for another user! (5) signing out and back in - it still took me to the old version every time.
